Student Specific Services
The Outreach Team can help you assess the needs of the deaf and hard of hearing students in your district and be a resource in providing appropriate services. Throughout the intake process, we will collaborate to identify student concerns prior to the on-site visit, identify common goals once the initial visit is complete, provide a report with recommendations, and develop a comprehensive plan of action.
In order to maximize the benefits of our collaboration the following is a list of documents we need completed before visits can be scheduled with your team:
- Student intake form
- Release of information form
- Student files (current IFSP/IEP, 504 Plan, evaluations, audiogram)
- Services agreement
Our team can provide program evaluation, which involves observation of the physical setting, student groupings, and the instructional model used. It also includes curriculum evaluation and both formative and summative assessment input.
